Insurance and Billing

We accept most insurances. It is expected that you familiarize yourself with your insurance policy and its benefits. If you are unsure whether a service or procedure is covered, it is recommended that you contact your insurance company prior to the date of service. The billing department cannot guarantee payment for services or quoted benefits from your health plan. You, as the patient are ultimately responsible for knowing your coverage limitations and benefits.

As a courtesy, we will bill your primary and secondary insurance for you; however, primary responsibility for the account is yours. Be sure to provide correct insurance billing information at the time of each appointment. If you do not have your insurance card with you at the time of your visit, and have not provided us with valid information, you will be billed for the services you have received.


For some uninsured patients we do accept cash at the time of your appointment. Call our office for a quoted price and details.


We do not require a co-pay at the time of the visit. We will bill your insurance first and if there is a co-pay, our billing department will send a bill in the mail. 


Payment is expected in full at the time of billing. Some rare cases may qualify for a payment plan not to exceed three months.  


We can accept payments over the phone or you are welcome to call our billing department at 503-579-5000.

If your visit requires additional services such as blood work, ultrasound or sending samples to an outside laboratory you will be receiving a separate bill from that company.
